Check the logo. White means a cosmetic defect. Dark means a park wiring problem.

Yes. This is a known cosmetic defect in serial numbers YW2001 to YW2321, caused by a faulty part on the display board. The unit still protects you. Keep using it, and call (888) 540-1504 or email warranty@autoformers.com with photos for a replacement.
No, the unit is dark except for the red LED. The park isn’t supplying a neutral. This is a pedestal or park wiring problem, not your RV. Try another pedestal and tell the park.
